Reminder: Your order will be voided if:
 
1. Your current address does not agree with address on renewal notice.
2. Your renewal requires an affidavit.
3. The name and address information on courtesy card does not match credit card name and billing address.
4. Your credit card is declined.
Other problems may occur, but you will be notified.

A $15 penalty will be added, plus 12% annualized interest to orders received after current month of renewal.
