The error on the Certificate of Title was due to the following:
Purchaser/Seller signed name in release of lien section in error.
Owner's name was misspelled on face of title.
VIN number is incorrect on face of title and should be corrected to read 
as follows:

Other than the above, please explain:

 

1.)    This affidavit can only be used to correct errors on the face of the
certificate of title.

2.)   This affidavit cannot be used to correct alterations on the title.

3.)    This affidavit cannot be used to correct any errors in the assignment
areas of the title or on any re-assignment supplements. Errors in an
assignment area can only be corrected through the use of a conforming
affidavit of correction (form MVT 5-7).
